Is it right for you?

Who azelaic acid + niacinamide is for

  • Dark spots
  • Marks after acne
  • Melasma
  • Redness

It may be a good fit if

  • You have dark spots, sun spots or marks left by breakouts
  • You have melasma, or patches that darken in the sun
  • Your skin flushes or looks red easily

Talk to your clinician first if

  • You are pregnant, trying to conceive or breastfeeding
  • You have very sensitive or broken skin on the area
  • You use other lightening products, such as hydroquinone
  • You notice lighter patches of skin after starting

Your clinician decides whether a prescription is appropriate for you, or recommends another option.

How it works

Two ingredients, two steps of the pigment process.

Dark spots form when pigment cells make too much melanin and pass it up to the surface of the skin. Azelaic acid is FDA-approved on its own (20% cream for acne, 15% gel for rosacea); this 15% cream with niacinamide is a compounded combination.

Azelaic acid 15%
Slows down overactive pigment-producing cells, without affecting normal skin color. It is also used for redness and breakouts.
Niacinamide 4%
A form of vitamin B3 that limits the transfer of pigment to the surface of the skin and supports the skin barrier.
One step, morning and evening
Both ingredients in a single cream, so your routine stays simple.
How to use it

A simple routine, set by your clinician.

  1. Morning and evening

Apply a thin layer to clean, dry skin over the whole area, not only the spots, as your clinician directs.

  2. Expect a light tingle

A mild, temporary tingling or stinging is common in the first weeks. Tell your clinician if it does not settle.

  3. Sunscreen every day

Sun is the main trigger for dark spots and melasma, so a broad-spectrum SPF 30 or higher every morning is part of the treatment.

  4. Give it time

Pigment changes slowly. Many people need 12 weeks or more of regular use. Your clinician checks in and adjusts your plan.

Always follow your clinician’s instructions and the pharmacy label. Individual results vary.

About compounded medications

How compounded medications work

Compounded medications are prescribed for an individual patient by a licensed clinician after a medical assessment, and prepared by a state-licensed 503A compounding pharmacy. They do not undergo FDA pre-market review or approval, and they may differ from FDA-approved drugs in effectiveness, safety and side effects. Results from clinical trials of FDA-approved medications should not be used to judge compounded medications.

Azelaic acid 20% cream is FDA-approved for mild to moderate acne and azelaic acid 15% gel or foam for rosacea; other uses are off-label. Azelaic acid 15% / niacinamide 4% cream is compounded by a state-licensed 503A pharmacy and is not FDA-approved.
